Youth Community Service and College Educational Scholarship

Please read all instructions carefully before submitting your application, video and budget. All documents submitted become the property of the Program Committee. It is the responsibility of the applicant to submit a complete proposal packet. The Matching-Grant Scholarship Program Committee will not assume responsibility for or attempt to compile materials submitted separately.

Instructions:

• The Matching-Grant Scholarship submission includes the following:

1) application,

2) video, and

3) proposed budget.

 • Applications will be submitted to and evaluated by the Matching-Grant Scholarship Program Committee. Deadline for submission is May 1  at 11:59 P.M. Central Standard Time.

• The video format can include mp3, mp4, or mov.  Mobile phone video is acceptable. The video should be up to 3 minutes max. (The video does need to be edited.)

• Videos will be evaluated based on how clearly you show the rationale of the need for the funding, program, creativity, action planning, collaboration and/or partnership(s) (if applicable), and the overall community impact.

• The submitted budget must: Include total funds for the scholarship/project/activity. Delineate the use of requested funds.  List funds from other sources or personal savings.

• Ten (10) Semi-finalists will be selected representing one member from each region. First, Second, Third place finalists will be announced by July 4.

• This is a one-time award.

• Award winners will submit a final report to the Program Committee that includes the actual budget expenditures, and accomplishments within 90 days of receipt of funding.

• Submit the 1) application, 2) video, and 3) proposed program budget.
